IDENTIFIKASI FAKTOR PENYEBAB KETIDAKLENGKAPAN PENGISIAN FORMULIR PENGKAJIAN AWAL MEDIS ELEKTRONIK PASIEN RAWAT JALAN DI UPT PUSKESMAS CIRUAS

Abstract

ABSTRACT
Background: Incomplete entry of electronic medical records in the initial medical assessment form can have a direct impact on the quality of healthcare services and medical documentation. Based on initial observations at the Ciruas Community Health Center (UPT Puskesmas Ciruas), it was found that the completeness of filling out the electronic initial medical assessment forms for new outpatient visits only reached 80%, which is still below the minimum standard of 100% set by the Ministry of Health.
Purpose: To identify the factors that contribute to the incompleteness in filling out the electronic initial medical assessment forms at UPT Puskesmas Ciruas.
Method: This study uses a descriptive qualitative approach. The study population includes all polyclinic staff at UPT Puskesmas Ciruas, totaling 32 individuals. The sample consists of the Head of Medical Records and officers responsible for completing the electronic initial medical assessment forms for outpatients in the general polyclinic, totaling 6 individuals.
Result: Based on interviews with informants, the factors causing the incompleteness in filling out the electronic initial medical assessment form for outpatients at UPT Puskesmas Ciruas include several aspects. From the Man aspect, some staff have less than five years of experience, and none can be considered fully experienced yet, as the e-Puskesmas system was only implemented on April 16, 2024, and not all staff have received equal technical training. From the Method aspect, there is no specific Standard Operating Procedure (SOP) available for the completion of the electronic assessment form. From the Material aspect, there are limited computers in the outpatient clinics and frequent internet instability, particularly during power outages, although these are rare and a backup generator is available at the Puskesmas
Conclusion: This study shows that incomplete completion of the electronic initial medical assessment form for outpatients at the Ciruas Community Health Center (UPT Puskesmas) is influenced by several factors, namely the Human (HR) aspect in the form of limited experience and uneven technical training, the Method aspect in the form of the unavailability of a specific SOP for filling, and the Material (Facilities and Infrastructure) aspect in the form of limited computer equipment and internet network disruptions. These findings indicate that improving the competence of officers through training, preparing clear SOPs, and strengthening supporting facilities are important steps to achieve the standard for completeness of filling in electronic medical records as stipulated by the Ministry of Health. This study contributes in providing an overview of the causal factors that can be the basis for improving the medical documentation system, so that the quality of health services at the Community Health Center can continue to be improved.

ABSTRAK
Latar Belakang: Ketidaklengkapan pengisian rekam medis elektronik pada formulir pengkajian awal medis dapat berdampak langsung terhadap mutu pelayanan dan dokumentasi medis. Berdasarkan observasi awal di UPT Puskesmas Ciruas, ditemukan bahwa kelengkapan pengisian formulir pengkajian awal medis elektronik pasien rawat jalan baru mencapai 80%, masih di bawah standar minimal 100% yang ditetapkan oleh kementerian kesehatan.
Tujuan: Untuk mengetahui faktor-faktor yang menyebabkan ketidaklengkapan pengisian formulir pengkajian awal medis elektronik pasien rawat jalan di UPT Puskesmas Ciruas.
Metode: Yaitu deskriptif kualitatif. Populasi subjek dalam penelitian ini adalah keseluruhan petugas poli yang ada di UPT Puskesmas Ciruas yang berjumlah 32 orang. Sampel dalam penelitian ini yaitu, Kepala Rekam Medis dan Petugas yang bertanggung jawab atas pengisian formulir pengkajian awal medis elektronik pasien rawat jalan di poli umum yang berjumlah 5 orang jadi keseluruhan sampel yaitu 6 orang.
Hasil: Berdasarkan wawancara dengan informan, Mengenai faktor penyebab ketidaklengkapan pengisian formulir pengkajian awal medis elektronik pasien rawat jalan di UPT Puskesmas Ciruas menunjukkan bahwa dari aspek Man (Manusia) berdasarkan pengalaman, Terdapat petugas dengan masa kerja di bawah 5 tahun dan semua petugas belum bisa dikatakan berpengalaman atau tidak karena system E-Puskesmas baru diterapkan pada tanggal 16 April 2024. Selain itu, belum semua petugas mendapatkan pelatihan teknis secara merata. Ditinjau dari aspek Method (Metode) berdasarkan ketersediaan SOP, belum tersedia Standar Operasional Prosedur (SOP) khusus terkait pengisian formulir pengkajian awal medis elektronik. Dari aspek Material (Sarana dan Prasarana), ditemukan keterbatasan jumlah perangkat komputer di poli serta kendala jaringan internet yang sering tidak stabil, khususnya saat terjadi pemadaman listrik dari pusat, sehingga proses pengisian menjadi terhambat namun jarang terjadi dan di UPT Puskesmas Ciruas tersedia genset.
Kesimpulan: Penelitian ini menunjukkan bahwa ketidaklengkapan pengisian formulir pengkajian awal medis elektronik pasien rawat jalan di UPT Puskesmas Ciruas dipengaruhi oleh beberapa faktor, yaitu aspek Man (SDM) berupa keterbatasan pengalaman dan belum meratanya pelatihan teknis, aspek Method (Metode) berupa belum tersedianya SOP khusus pengisian, serta aspek Material (Sarana dan Prasarana) berupa keterbatasan perangkat komputer dan gangguan jaringan internet. Temuan ini mengindikasikan bahwa peningkatan kompetensi petugas melalui pelatihan, penyusunan SOP yang jelas, serta penguatan sarana pendukung merupakan langkah penting untuk mencapai standar kelengkapan pengisian rekam medis elektronik sesuai ketentuan Kementerian Kesehatan. Penelitian ini berkontribusi dalam memberikan gambaran faktor penyebab yang dapat menjadi dasar perbaikan sistem dokumentasi medis, sehingga mutu pelayanan kesehatan di Puskesmas dapat terus ditingkatkan.
